Financial Policy:

Our office participates with most of the major insurance companies. Even though we may participate with an insurance company, patients still have certain responsibilities. Please contact our office if you have any questions concerning our financial policies.
Patients are responsible for:
-
Bringing a current insurance card and ID card to each visit. If a patient comes into the office without his or her insurance card, they will be responsible for the full payment of that day's charges.
-
Understanding the basic requirements of his or her insurance policy.
-
Paying any co-pay, unmet deductible, or co-insurance at the time of the visit.
-
Obtaining a referral from the primary care physician if a referral is required by their insurance policy before a patient can be seen by a specialist.
-
Notifying our office of any change in insurance coverage. If the insurance company has changed or the patient has received a new card from the same insurance company, our office must be notified.
-
Cancellations: Cancellations made with less than 1 business days notice may be subject to a cancellation fee of $50. Last minute cancellations of a surgical appointment may be subject to a fee of $100.
-
Medical Records: We reserve the right to charge for medical record requests in accordance with the fee structure as set forth in the North Carolina state statute. You will be responsible for paying this fee prior to mailing or pick-up of these records.
