Cancellation Policy
Our office requires at least 24 hour notice of any appointment cancellations. If our office is closed, please leave a voicemail with your name, date of birth, and appointment date that you would like to cancel. If you are a current patient, after three same day cancellations (barring extenuating circumstances) you will be discharged from the practice. If you are a new patient, after one same day cancellation (barring extenuating circumstances) you will be discharged from the practice.
If you are a current patient that fails to show up at the correct appointment date and time more than two times you will be discharged from the practice. If you are a new patient and fail to show up at the correct appointment date and time more than once you will be discharged from the practice.
If you are a new patient and have failed to fill out your online intake paperwork you must come to your appointment at least 5 minutes early. If you fail to do so we will be forced to cancel your appointment and reschedule to a later date.
Patients who fail to confirm their appointment through either text message or phone call by the end of the business day prior to their appointment date may have their appointment subject to cancellation.
